National Priorities Partnership (NPP) Care Coordination Convening Workshop 

The National Priorities Partnership (NPP) Care Coordination Convening Workshop will be held on September 1-2, 2010, at the Hyatt Regency Washington, located at 400 New Jersey Avenue, NW, in Washington, DC.

The aim of this meeting is to focus on the timeliness of patient follow-up immediately following hospital discharge and develop recommendations to present to the NPP Partners and other stakeholder groups that:

• Identify environmental barriers to achieving the NPP Care Coordination goals and a plan to address these barriers, including specific actions that Partners and other stakeholders can take by focusing on identified drivers;
• Identify gaps in measurement and a plan for filling high-priority gaps; and
• Address implications for health information technology (HIT).

The meeting is open, but registration is required, as space is limited. If you cannot attend in-person or miss the registration deadline, a call-in line will be available. Meeting materials will be available to the public in the coming weeks.
