I’m entering sensitive data into the online form. How can I be sure that no one else will be able to access this information?
Each online submission form is tied directly to the website account of the person who creates the
draft of that document. That is, if you begin to enter data into the form, and save a draft to return
to later, NO OTHER USER will have access to the information that you’ve entered unless you grant permission.

Completing the Form
I entered all of my information
to answer the question, but my response overfills the text box. Is my answer cut off? How can I be
sure everything is in there?
No, your answer is not trimmed. To be sure your information is present, choose the Print Submission
link. Doing so will open a pop-up, print-ready version of your current form, including all of the
data that you’ve entered. Scrull down to the field you want to check, and you'll see all your
information is present.

I’d like to submit more than one
measure to this NQF project. Can I use the same online form to submit data about more than one
measure?
No. You must complete a separate online submission form for each measure that you plan to submit to
NQF for consideration.

Do I have to complete the tabs in
order?
No, you can enter data into the form in any order
that you wish.

Which document formats are
supported by the online submission form, and what’s the size limit for attachments?
A few questions request or allow an attachment and a specific format may be indicated. The online form supports Word, Excel, Power Point, and PDF documents. Attached documents can be no
larger than 5MB.

There is more information that I
want to submit, and no place on the form for it. What do I do?
The form has a place where you can attach an appendix of supplemental materials. However, all requested information should be provided in the submission form and evidence and measure testing attachments. There is no guarantee that supplemental materials will be reviewed.

Saving Drafts
How do I save a draft version of my
submission form?
The online form will not automatically save your work. To save your work, you must click the Save Draft link in the upper right corner of the form or the Save Draft button at the bottom of each tab.

Where can I find the date and time of my
last successful save of my draft form?
Check the time and date of your last successful save attempt displayed at the top of each page of the
form. This information is located just below the “NQF Measure Submission Version X.X” header.

How can I reopen a draft submission form
that I’ve saved?
You can access your drafts through your Dashboard. Log in to the NQF site, and open your Dashboard.
In the Dashboard, just below your contact information, you’ll see a section called Submissions. Within
this section, you’ll see a list of the online submission forms that you’ve begun but haven’t yet
submitted. To access one of the drafts, click on its title.

How can I print a hard copy of the online
form for my records?
Click on the Print Submission link at any time during your work with the online submission
form. A pop-up window will appear with full print-ready version of your form. Any information that
you’ve entered into the form will be present in this printed version.

Can I save an electronic version of the online form for my records?
If your system allows, you can print to a PDF document and save the file.

Submitting the Online Form
How do I submit an online
submission form that I’ve completed?
To submit your measure, click the Submit link located in the upper right-hand or the Submit button at the bottom of each tab of the submission form. The website will run a basic validation check. If it
discovers any errors, a pop-up window will appear with a linked list of those required areas that you
still need to complete.
If there are no errors, the Submission pop-up will appear. At this point, you have
not yet submitted your measure to NQF. To complete the submission process, you will need to
certify within the pop-up window that you’ve completed the submission form and entered all of the
required information. Once you’ve made this certification, and when you’re certain that you’re ready,
click the Ok button to submit your measure.

Can I make any changes to my form
once I’ve submitted it to NQF?
No. Once you’ve submitted your form, you will not be able to make any further changes
to your submission form unless NQF staff reopen the form. Contact the project staff if you need to make any changes after you have submitted your measure.

Where can I see a list of the
online forms that I’ve submitted?
This information can be found on your Dashboard. Log in to the NQF website and go to Membership > My Dashboard. In
the Dashboard, just below your contact information, you’ll see a section called Submissions. Within
this section, you’ll see a list of the forms that you’ve submitted and those still in draft.
