Multiple Chronic Conditions (MCC) Measurement Framework 

NQF hosted a public webinar on Monday, March 5 at 1:00 pm ET to discuss the Multiple Chronic Conditions (MCC) Measurement Framework report. Following endorsement through NQF’s consensus development process, this framework will serve as a guide for future NQF-endorsement decisions for performance measures that address this population. The framework, developed by NQF under the direction of a multi-stakeholder MCC Steering Committee, establishes a definition for MCCs, identifies high-leverage measurement areas, presents a conceptual measurement model, offers guiding principles for facilitating quality of care for individuals with MCCs, and highlights strategic opportunities for applying the framework.
