NQF’s
Action Teams are diverse, public-private multistakeholder partnerships that aim
to create positive change through collaboration and collective impact. NQF has successfully used the Action Team model to address
significant health and healthcare challenges. In 2012-2014, NQF’s Maternity
Action Team contributed to a more than 50-percent drop in national early
elective delivery rates.
The 2014 Readmissions Action Team supported the spread of best practices to
identify and manage patients at high risk for readmissions due to psychosocial
issues. NQF also developed an interactive patient engagement tool, the Patient Passport,
through the Patient and Family Engagement Action Team to help patients
communicate better with their doctors and care teams about their needs, values,
and preferences.
The 2015 Action Teams will conduct focused year-long efforts
to promote awareness and accountability for the healthcare system in antibiotic
stewardship as well as high-quality, person-centered care for people with serious
advanced illness. Through webinars and in-person meetings, the teams will bring
attention to and broadly disseminate best, promising, or innovative practices;
promote solutions to improve appropriate antibiotic prescribing and deliver of high-quality
advanced illness care; and partner to implement robust strategies such as
performance measurement, public reporting, payment, accreditation, and
certification.
