Comments on the Common Formats Readmissions Version 0.1 Beta and Version 1.2 (Hospitals) 

NQF is currently receiving comments on Readmissions Common Format-Version 0.1 Beta and Common Formats Version 1.2 (acute care hospitals).

The comment period (60 days) for the Readmissions-Version 0.1 Beta Common Format will close on September 20 at 6:00pm Eastern Time.

The comment period for Common Formats Version 1.2 will remain open until Version 2.0 is released. The NQF Expert Panel will consider the comments received through August 10, 2012 in its initial set of recommendations to AHRQ. Commenters are encouraged to submit comments by August 10 to ensure they can be fully considered by AHRQ in development of Version 2.0.

NQF, on behalf of the Agency for Healthcare Research and Quality (AHRQ), is coordinating a process to obtain comments from stakeholders about the Common Formats authorized by the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act). The Common Formats provide a standardized method for data collection and reporting of patient safety and quality information to federally-listed Patient Safety Organizations (PSOs).

The tool that you are about to access was developed by NQF to collect comments on the Common Formats event descriptions. This NQF commenting tool includes the content of each of the currently available Common Formats event descriptions and forms (listed on the first page of the tool). In addition to accessing the event descriptions for comment, relevant segments of the related Common Formats reporting forms are displayed for context and comment.

Comments on Readmissions - Version 0.1 Beta and Version 1.2 (acute care hospitals) and may be submitted at any time until their comment periods close.   

  • The closing date for comments on the Readmissions - Version 0.1 Beta Common Format is September 20. NOTE: The Comment Period is open for 60 days. 
  • The closing date for comments on Common Formats Version 1.2 (acute care hospitals) will coincide with the release of Version 2.0. 
  • The NQF Expert Panel will consider comments received during the initial 120 days of the comment period in late summer/early Autumn 2012. Comments received after this period will be considered at regular intervals. 

Users of this site will be advised of postings of new versions of the Common Formats as they are released.

Please keep in mind this tool allows the user to comment on the Common Formats and is not intended to allow submission of actual patient safety events. As an example, for the item "Documentation of whether the fall was observed" -- the user may comment on the content and wording of the item, but the user should not enter comments using patient information.
