Submission of this completed form will serve as notice to NQF that your organization intends to apply for the 2008 Award.  Within five business days of submission, NQF will e-mail your designated contact person a unique identifier that will serve as your application identification number and should be used when preparing the blinded version of the organization’s application.  Submission of this form only serves as notification to NQF that you intend to apply, it does not bind you to doing so.
