National Quality Forum Spring Conference
Leaders in Healthcare Quality to focus on National Priorities and Care Coordination

WHAT: National Quality Forum Implementation Conference on Care Coordination and 2008 Spring Membership Meeting. A full agenda is available at www.qualityforum.org.

March 26: Spring Membership Meeting. The membership meeting will be a dialogue between NQF membership and leaders of the National Priorities Partners, a group of 27 organizations convened by NQF that is identifying a short list of “high-impact” national priorities for measurable improvement in America’s healthcare system.

March 26-28: Implementation Conference on Care Coordination. Sessions will focus on innovative payment approaches to facilitate care coordination and patient engagement, as well as:

  • the healthcare "home",
  • proactive plan of care and follow-up that anticipates needs and tracks progress,
  • communications available to all team members including patients and families,
  • the use of standardized integrated electronic information systems with functionalities essential to care coordination, and
  • transitions (or "hand-offs") between settings of care.

WHO: NQF’s almost 400 members are leaders from all parts of the healthcare system, including national, regional, state, and local groups representing consumers; public and private purchasers; physicians, hospitals, and other healthcare providers; accrediting bodies; supporting industries; and organizations involved in healthcare research/quality improvement.
