Abstract
Primary care plays an important role in improving the health of individuals and populations. As the population in the United States ages, the number of those living with chronic illnesses, disability and functional limitations has increased. To manage a patient’s health, primary care practitioners should integrate the continuity of care process into their practice, as this allows patients and their practitioners to manage their care together throughout the disease state. During this measure review cycle, the Primary Care and Chronic Illness Standing Committee evaluated one new measure related to continuity of care based on NQF’s standard evaluation criteria. The Standing Committee recommended the measure for endorsement and the Consensus Standards Approval Committee upheld the Standing Committee’s recommendation.
