Please join NQF’s National Quality Partners™ (NQP™) Social Determinants of Health (SDOH) Data Integration Action Team for a webinar on Thursday, July 11, 1:00 – 2:00pm ET. NQP Action Team members will discuss leading practices and recommendations to accelerate the integration of SDOH data into care.
NQP Action Team members will highlight how diverse organizations can collaborate and take action to improve SDOH data integration. Speakers will share examples of standardizing SDOH data, engaging individuals and communities in data collection, overcoming barriers in data interoperability, and improving clinical and community partnerships. The NQP Action Team panel will emphasize the importance of multistakeholders playing a role in overcoming the challenges to integrating SDOH data into practice.
