MAP Post-Acute Care/Long-Term Care Workgroup

Overview

The National Quality Forum (NQF) Measure Applications Partnership (MAP) convened a MAP Post-Acute Care Long-Term Care (PAC/LTC) Workgroup on behalf of the Centers for Medicare & Medicaid Services (CMS) to provide input on the measures under consideration (MUC).

The Opportunity

In partnership with the Centers for Medicare & Medicaid Services (CMS), the National Quality Form (NQF) convened a MAP PAC/LTC Workgroup to provide input on the measures under consideration (MUC) to the U.S. Department of Health and Human Services (HHS) with a lens for selection and coordination of measures for PAC/LTC providers, including hospices, inpatient rehabilitation facilities, long-term care hospitals, skilled nursing facilities, and home health care.

About the Project

In this project, the MAP PAC/LTC Workgroup provides input to the Coordinating Committee on matters related to post-acute care and long-term care programs. Its aim is to establish performance measurement alignment across PAC/LTC settings while emphasizing that alignment must be balanced with consideration for the heterogeneity of patient needs across settings. This is achieved by acknowledging the distinct types of care and levels of care across PAC and LTC settings and identifying measures that can address these types and levels of care while also considering the multiple provider types with varying payment structures.


Objective

The PAC/LTC Workgroup provides annual pre-rulemaking input on the following programs:

  • Home Health Quality Reporting Program (HH QRP)
  • Skilled Nursing Facility Quality Reporting Program (SNF QRP)
  • Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP)
  • Long-Term Care Hospital Quality Reporting Program (LTCH QRP)
  • Hospice Quality Reporting Program (HQRP)
  • Skilled Nursing Facility Value-Based Purchasing (SNF VBP) Program
