Care Coordination Convening Meeting Synthesis Report

Abstract
In its 2008 report, National Priorities and Goals—Aligning Our Efforts to Transform America’s Healthcare, the National Priorities Partnership (NPP) identified six National Priorities that if addressed would significantly improve the quality of care delivered to Americans. The Care Coordination Priority aims to ensure that patients receive well-coordinated care within and across all healthcare organizations, settings, and levels of care. In September 2010, the Care Coordination Workgroup convened to identify specific actions NPP partners and others could consider – action that if implemented would have the greatest potential to help healthcare organizations and staff work collaboratively with patients to reduce 30-day readmission rates.
