Readmissions Action Pathway 

For its work in support of the Partnership for Patients initiative, NQF’s multistakeholder Readmissions Action Team developed an action pathway with a primary aim of promoting person-centered care for vulnerable populations to safely reduce avoidable admissions and readmissions. Action team members are championing strategies and spreading resources to identify high-risk patients with psychosocial needs, and to leverage patient, provider, and community partnerships to address those needs. This work complements the Partnership for Patients’ goal of reducing hospital readmissions and the many efforts focused on improving care transitions from the hospital setting.
