Abstract
This is the third in a series of reports describing NQF's 2014-2016 measure evaluation project for person- and family-centered care measures. Ensuring that all persons and their families are engaged as partners in care is one of the six priorities of the National Quality Strategy. This includes shared decision-making and preparation and activation for self-care management. This off-cycle project focused on patient activation and review of annual updates on previously endorsed measures. The Committee reviewed one new measure and examined the finalized risk-adjustment strategies for two measures that had received conditional endorsement in Phase 2 of the PFCC work.
