Essential Attributes of a High-Quality System of Care - How Communities Approach Quality Measurement

Abstract
In a new report, National Quality Forum (NQF) highlights case studies of how communities are addressing the attributes described by The SCAN Foundation as being essential for a high-quality healthcare system for adults with complex care needs. NQF found that the first essential attribute—to identify and regularly re-evaluate a person’s medical and nonmedical needs and goals on an ongoing basis for care planning—is the one communities are most consistently achieving by using data, measurement tools, and instruments in innovative ways.
