Leveraging Electronic Health Record EHR-Sourced Measures to Improve Care Communication and Coordination - Final Recommendations Report 

In 2021, under a contract with the Centers for Medicare & Medicaid Services (CMS), the National Quality Forum (NQF) convened a multi-stakeholder Committee to identify best practices to leverage EHR-sourced measures to improve care communication and care coordination quality measurement. Building on the environmental scan findings from the first phase of work, the Committee developed five recommendations to support the evolution of EHR-based care communication, care coordination, and performance measurement to drive quality improvement and equitable health outcomes: collect and share standardized data, optimize EHR usability for patients and caregivers, optimize EHR usability for clinicians, develop novel EHR data elements to improve measurement, and leverage EHR data to fill measurement gaps.This Final Recommendations Report provides extensive detail about the recommendations and is designed for measure developers, EHR vendors, healthcare providers, and other quality measurement stakeholders. Another version of the report, the Shortened Final Recommendations Report, provides a high-level overview of the recommendations for changemakers, policy and legislative professionals, clinicians, patient advocates, and members of the public.
