Improving Population Health - Short Story
NQF Launches Three-Year Population Health ProjectExisting U.S. programs and policies that improve wellness and healthy behaviors across populations have resulted in estimated health care savings of $19 billion over 10 years, according to The Commonwealth Fund. Given this promise, NQF is working on a three-year, HHS funded project to amplify and expand population health initiatives nationally.The first phase is the draft Community Action Guide, a new resource designed to help communities initiate or improve their population health programs. Developed by an NQF-convened, multi-stakeholder committee, the draft Guide is based on research about what works to improve population health. "We saw a need for a step-by-step guide to help communities meet their healthcare goals," said Kaye Bender, PhD, RN, co-chair of the Population Health Framework committee. "The Action Guide offers help through plain language guidance."
The guide--recently circulated for public comment--features 10 elements important in working to improve population health, along with suggested actions and links to practical resources. The project’s second phase will engage “feedback communities” to help refine the guide.
NQF’s other population health work includes the Health and Well-Being Measures project, the MAP Population Health Task Force and endorsed measures addressing clinical preventive services and immunizations,as well as healthy behaviors and broader population-level measures.
