NQF Launches Social Determinants of Health (SDOH) Data Integration Action TeamForty-one NQF Members participating in Action Team

We are pleased to launch the National Quality Partners™ (NQP™) Social Determinants of Health (SDOH) Data Integration Action Team to bring experts and recognized leaders together to stimulate shared learning and action to accelerate SDOH data integration into clinical practice. While research has shown that the conditions that people work, learn, play, and live, otherwise referred to as SDOH, account for 40% health outcomes, data on SDOH has largely been left out of clinical practice. Thus, there is a need for a national, concerted effort to lead the integration of SDOH data into the health care delivery system and capture data in a way that is actionable in order to reduce disparities and improve healthcare access and health outcomes of patients and communities.

By partnering together and sharing best practices, the NQP SDOH Data Integration Action Team will identify solutions to common pain points related to SDOH data integration. Through a series of web meetings and one in-person forum, the NQP Action Team will foster collaboration and sharing to address gaps in care and inspire action in others.

In order to benefit from diverse viewpoints and experiences, the NQP SDOH Data Integration Action Team spans organizations across all sectors of health care including, patients, caregivers, clinicians, physician associations, research groups, federal agencies, and more.
