Join National Quality Partners (NQP) for a virtual forum on Wednesday, March 15, 1:00 – 2:30pm ET, to discuss how measures can serve as a practical tool to improve person-centered care delivery across various healthcare settings.
This virtual forum will follow Hazel, a 63-year-old woman diagnosed with advanced lung cancer, as she transitions through the healthcare system, from inpatient and post-acute care to palliative and hospice care. During her journey, see how quality measures can address issues that are important to Hazel’s quality of life and support the delivery of care that reflects her personal preferences, values, and goals.

Hazel’s case illustrates how measures can support the six key preferences of person-centered advanced illness care identified in NQP’s issue brief, A Collaborative Journey to Transform Advanced Illness Care: purpose and connection, physical comfort, emotional and psychological well-being, family and caregiver support, financial security, and peaceful death and dying. Developed by NQP’s Advanced Illness Care Action Team—which includes patient advocates, physicians, hospital systems, and other stakeholders from the public and private sectors—the issue brief explores these key preferences in depth, along with snapshots of organizations that have embraced one or more of preferences.
