1 - Adolescent Preventive Care
Organization: New York State Department of Health
Description: Adolescent preventive care measures address risk behaviors most commonly associated with morbidity and mortality for adolescents and young adults. The provision of preventive care involving these areas is recommended for all adolescents. The adolescent preventive care measures reflect the rates at which adolescents are interacting with health care providers regarding the topics during visits.

The percentage of adolescents ages 12 to 17 who had at least one had an outpatient visit with a PCP or OB/GYN practitioner during the measurement year, receiving the following components of care during the measurement year:

1. Assessment or counseling or education on risk behaviors and preventive actions associated with sexual activity,
2. Assessment or counseling or education for depression,
3. Assessment or counseling or education about the risks of tobacco usage, and
4. Assessment or counseling or education about the risks of substance use (including alcohol and excluding tobacco).

The four components are reported separately as individual rates for the components.

2 - Risky Behavior Screening

Description: The percentage of adolescents who turned 18 years old during the measurement year and who had a risky behavior screening and proper follow-up performed during the two years prior to the child's 18th birthday.

Description: The percentage of children who had an oral health exam and proper follow-up performed. Four rates are reported:
1. By 2 years of age
2. By 6 years of age
3. By 13 years of age
4. By 18 years of age

Description: The percentage of children who had a vision screening and proper follow-up. Three rates are reported:
1. By age 6 years

Description: The percentage of adolescents who had proper immunizations. Two rates are reported:

Description: The percentage of children who had a BMI assessment and counseling for physical activity, nutrition, and screen time. Three rates are reported:

Description: The percentage of children 2 years of age who had one or more venous lead blood test for lead poisoning by their second birthday.

Description: The percentage of children who turned 6 months old during the measurement year and who had the following indicators of comprehensive well care documented between birth and six months of life.

Description: The percentage of children who turned two years old during the measurement year and who had the following indicators of comprehensive well care documented between 6 months and 2 years of life.

Description: The percentage of adolescents who had a depression screening and proper follow-up. Two rates are reported:

Description: The percentage of members 2–21 years of age who had at least one dental visit during the measurement year.

15 - Adolescent Well-Care Visits

Description: The percentage of enrolled members 12–21 years of age who had at least one comprehensive well-care visit with a PCP or an OB/GYN practitioner during the measurement year.

16 - Child and Adolescents’ Access to Primary Care Practitioners
Organization: National Committee for Quality Assurance
Description: The percentage of members 12 months–19 years of age who had a visit with a PCP. The organization reports four separate percentages for each product line.
- Children 12–24 months who had a visit with a PCP during the measurement year
- Children 25 months–6 years who had a visit with a PCP during the measurement year
- Children 7–11 years who had a visit with a PCP during the measurement year or the year prior to the measurement year
- Adolescents 12–19 years who had a visit with a PCP during the measurement year or the year prior to the measurement year

17 - Frequency of Ongoing Prenatal Care
Organization: National Committee for Quality Assurance
Description: The percentage of Medicaid deliveries between November 6 of the year prior to the measurement year and November 5 of the measurement year that received the following number of expected prenatal visits.
•<21 percent of expected visits
•21 percent–40 percent of expected visits
•41 percent–60 percent of expected visits
•61 percent–80 percent of expected visits
•=81 percent of expected visits
This measure uses the same denominator as the Prenatal and Postpartum Care measure.

18 - Prenatal & Postpartum Care
Organization: National Committee for Quality Assurance
Description: The percentage of deliveries of live births between November 6 of the year prior to the measurement year and November 5 of the measurement year. For these women, the measure assesses the following facets of prenatal and postpartum care.
•Timeliness of Prenatal Care. The percentage of deliveries that received a prenatal care visit as a member of the organization in the first trimester or within 42 days of enrollment in the organization.
•Postpartum Care. The percentage of deliveries that had a postpartum visit on or between 21 and 56 days after delivery.

19 - Well-Child Visits in the First 15 Months of Life

Description: The percentage of members who turned 15 months old during the measurement year and who had the following number of well-child visits with a PCP during their first 15 months of life.
