1 - Young Adult Health Care Survey (YAHCS)

Description: 54-item survey given to teenagers that assesses whether young adults (age 14 and older) are receiving nationally-recommended preventive services.

2 - Well-Child Visits in the Third, Fourth, Fifth, and Sixth Years of Life

Description: The percentage of members 3–6 years of age who received one or more well-child visits with a PCP during the measurement year.

3 - Well-Child Visits in the First 15 Months of Life

Description: The percentage of members who turned 15 months old during the measurement year and who had the following number of well-child visits with a PCP during their first 15 months of life.

Description: Assesses if children have received any treatment or counseling from a mental health professional in the past 12 months

7 - Unmet needs for specialist care

Description: Measures how many children needed to see a specialist but did not receive specialist care in the past 12 months

8 - Sudden Infant Death Syndrome Counseling

Description: The percentage of children who turned 6 months old during the measurement year and who had Sudden Infant Death Syndrome (SIDS) counseling and proper follow-up between 0 months and 6 months of life.

9 - Shared decision making

Description: Measures whether parent is actively engaged as a partner in child's care (satisfaction with care and engagement in decision-making)

10 - Risky Behavior Screening

Description: The percentage of adolescents who turned 18 years old during the measurement year and who had a risky behavior screening and proper follow-up performed during the two years prior to the child's 18th birthday.

11 - Referral to intervention within 48 hours (EHDI-4b)
Organization: Centers for Disease Control and Prevention (CDC)
Description: This measure assesses the percent of infants and young children referred to intervention within 48 hours of the confirmation of permanent hearing loss.

12 - Provider engages parent around developmental health concerns

Description: Assesses quality of care by asking parents if their doctor specifically asked of any concerns they have with their child's (age 0-5 years) development

13 - Promoting Healthy Development Survey (PHDS)

Description: 43-item survey given to parents of children ages 3 to 48 months that assesses parent’s experience with care for the provision of preventive and developmental services consistent with American Academy of Pediatrics and Bright Futures practice guidelines.

14 - PREVENTIVE SERVICES — CHILD

Description: The rates represent the percent of enrolled members ages 18 and younger by end of measurement period, who are up-to-date (UTD) for all appropriate preventive services and the up-to-date rate by each service type. The measure includes preventive screening appropriate to each member’s age and gender including immunizations and vital statistics.

17 - Prenatal & Postpartum Care

Description: The percentage of deliveries of live births between November 6 of the year prior to the measurement year and November 5 of the measurement year. For these women, the measure assesses the following facets of prenatal and postpartum care.
•Timeliness of Prenatal Care. The percentage of deliveries that received a prenatal care visit as a member of the organization in the first trimester or within 42 days of enrollment in the organization.
•Postpartum Care. The percentage of deliveries that had a postpartum visit on or between 21 and 56 days after delivery.

Description: Whether child has one or more doctors, nurses or other healthcare providers who know the child well

19 - Parent report of BMI status

Description: Age and gender specific calculation of BMI based on parent reported height and weight of child

20 - Outpatient hearing screening of infants who did not complete screening before hospital discharge (EHDI-1c)
Organization: Centers for Disease Control and Prevention (CDC)
Description: This measure assesses the percent of infants not screened at birth who receive outpatient screening by 30 days of age.
