1 - Hearing screening prior to hospital discharge (EHDI-1a)

Description: This measure assesses the percentage of births that have been screened for hearing loss before hospital discharge.

2 - Hearing Screening refer rate at hospital discharge (EHDI-1b)

Description: This measure assesses the percentage of all newborn infants who fail initial screening and fail any subsequent re-screening before hospital discharge.

3 - Outpatient hearing screening of infants who did not complete screening before hospital discharge (EHDI-1c)

Description: This measure assesses the percent of infants not screened at birth who receive outpatient screening by 30 days of age.

4 - Infants identified with risk factors within the Medical Home (EHDI-2a)

Description: This measure assesses the percent of infants in a practice that have completed risk factor analysis for delayed onset or progressive hearing loss within the first year of life.

5 - Infants identified with risk factors and have an audiological diagnosis (EHDI-2b)

Description: This measure assesses the percent of young children in a practice that have an identified risk factor for delayed onset or progressive hearing loss and have an audiological diagnosis.

6 - Audiological Evaluation no later than 3 months of age (EHDI-3)

Description: This measure assesses the number of newborns who did not pass hearing screening and have an audiological evaluation no later than age 3 months of age.

7 - Intervention no later than 6 months of age (EHDI-4a)

Description: This measure assesses the percentage of infants with a permanent hearing loss who have been referred to intervention services no later than age 6 months of age.

8 - Referral to intervention within 48 hours (EHDI-4b)

Description: This measure assesses the percent of infants and young children referred to intervention within 48 hours of the confirmation of permanent hearing loss.

9 - Annual Dental Visit

Description: The percentage of members 2–21 years of age who had at least one dental visit during the measurement year.

10 - Adolescent Well-Care Visits

Description: The percentage of enrolled members 12–21 years of age who had at least one comprehensive well-care visit with a PCP or an OB/GYN practitioner during the measurement year.

11 - Child and Adolescents’ Access to Primary Care Practitioners

Description: The percentage of members 12 months–19 years of age who had a visit with a PCP. The organization reports four separate percentages for each product line.
- Children 12–24 months who had a visit with a PCP during the measurement year
- Children 25 months–6 years who had a visit with a PCP during the measurement year
- Children 7–11 years who had a visit with a PCP during the measurement year or the year prior to the measurement year
- Adolescents 12–19 years who had a visit with a PCP during the measurement year or the year prior to the measurement year

12 - Frequency of Ongoing Prenatal Care

Description: The percentage of Medicaid deliveries between November 6 of the year prior to the measurement year and November 5 of the measurement year that received the following number of expected prenatal visits.
•<21 percent of expected visits
•21 percent–40 percent of expected visits
•41 percent–60 percent of expected visits
•61 percent–80 percent of expected visits
•=81 percent of expected visits
This measure uses the same denominator as the Prenatal and Postpartum Care measure.

13 - Prenatal & Postpartum Care

Description: The percentage of deliveries of live births between November 6 of the year prior to the measurement year and November 5 of the measurement year. For these women, the measure assesses the following facets of prenatal and postpartum care.
•Timeliness of Prenatal Care. The percentage of deliveries that received a prenatal care visit as a member of the organization in the first trimester or within 42 days of enrollment in the organization.
•Postpartum Care. The percentage of deliveries that had a postpartum visit on or between 21 and 56 days after delivery.

14 - Well-Child Visits in the First 15 Months of Life

Description: The percentage of members who turned 15 months old during the measurement year and who had the following number of well-child visits with a PCP during their first 15 months of life.

Description: The percentage of children who turned 2 years old during the measurement year who had an autism screening and proper follow-up performed between 6 months and 2 years of life.

Description: The percentage of children who had a blood pressure screening and proper follow-up performed. Three rates are reported:
