Health Information Technology to Support Care Coordination and Care Transitions 

This commissioned paper gives an overview of health information technology (IT) to support care coordination and care transitions, starting with background on existing measures and legislation governing health IT. The paper goes on to describe data needs for care coordination, and addresses the current capabilities of clinical information systems to fulfill these data needs. It also outlines both organizational and technical barriers to improving the capabilities of heath IT and health information exchange, and puts forth approaches to addressing each barrier.
