Abstract
The Final Pathway for the Patient and Family Engagement Action Team summarizes the work of the action team to reach its goal of increasing authentic partnership between patients, families, and healthcare teams. During the course of its work, the action team developed a Patient Passport model, intended as a communications tool to help patients express their needs and preferences with providers at the point of care. The pathway describes plans to spread patient-preferred practices through partnerships with hospitals, health systems, and associations of health care providers, and to activate patients around the use of the passport. The work of the Patient and Family Engagement Team is funded by the Partnership for Patients in support of their goals of reducing preventable hospital readmissions and hospital-acquired conditions.
