Abstract
The Priority Setting for Healthcare Performance Measurement: Addressing Performance Measure Gaps in Care Coordination report offers multistakeholder recommendations to fill performance measurement gaps in care coordination. The report’s conceptual framework highlights the need for improved coordination between primary care settings and community-based services. The report describes the Committee’s recommendations for measure development in four high-impact areas: linkages and synchronization, individuals’ progression toward goals, comprehensive assessment, and shared accountability.
