RARE Campaign Cuts Readmissions Across Minnesota

For patients who have spent days in the hospital receiving treatment, the ability to go home and heal in a familiar and comfortable setting can make a big difference in their recovery.
A consortium of Minnesota-based companies including the Institute for Clinical Systems Improvement, Minnesota Hospital Association, and Stratis Health recognized the importance of healing at home and established the Reducing Avoidable Readmissions Effectively (RARE) Campaign. Submissions for the 2014 John M. Eisenberg Patient Safety and Quality Award will be accepted August 1 through September 30, 2014.
