Comments on the Common Formats Version 1.1 and Beta version of the Venous Thromboembolism (VTE) Common Format

NQF is currently receiving comments on Version 1.1 (acute care hospitals) of the Common Formats.

The comment period for Common Formats Version 1.1 (acute care hospitals) will remain open until an updated version is posted. As previously announced on this site, an initial set of comments collected through 6 pm ET on December 31, 2010 has been provided to AHRQ to facilitate development of Version 1.2. Comments received subsequent to that date will be provided to AHRQ to facilitate development of future versions.

NQF, on behalf of the Agency for Healthcare Research and Quality (AHRQ), is coordinating a process to obtain comments from stakeholders about the Common Formats authorized by the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act). The Common Formats provide a standardized method for data collection and reporting of patient safety event information to federally-listed Patient Safety Organizations (PSOs).

The tool that you are about to access was developed by NQF to collect comments on the Common Formats event descriptions. This NQF commenting tool includes the content of each of the currently available Common Formats event descriptions and forms (listed on the first page of the tool). In addition to accessing the event descriptions for comment, relevant segments of the related Common Formats reporting forms are displayed for context and comment.

Instructions for first time users: please create an account using the “Create account” button in the box on the right. You will then receive an e-mail with instructions to activate your account. If you do not receive an email within 5 minutes, please check your spam folder. When you log in for the first time, we will orient you to the tool with a brief (90 second) tutorial. If you have already created an account for this tool, you may login using the box at right.

Comments on Version 1.1 (acute care hospitals) Common Formats may be submitted at any time until the comment periods close.  

 

  • The closing date for comments on Common Formats Version 1.1 (acute care hospitals) will coincide with the release of Version 1.2 (anticipated in mid – 2012). 

 

Users of this site will be advised of postings of new versions of the Common Formats as they are released. 

Please keep in mind this tool allows the user to comment on the Common Formats and is not intended to allow submission of actual patient safety events. As an example, for the item "Documentation of whether the fall was observed" -- the user may comment on the content and wording of the item, but the user should not enter comments using patient information.

Specifically, you may comment on the Common Formats in their entirety or you may comment on any item of the event descriptions and related reporting forms. You may comment on any part of the Common Formats event description displayed on the screen simply by selecting it. Also, you may comment on item(s) on displayed sections of the reporting form. You may even comment on the instructions given for each event description. All feedback is valuable; please enter as many comments as you like.
