In 2002, the National Quality Forum (NQF) published Serious Reportable Events in Healthcare: A Consensus Report. Since then several healthcare error reporting systems have been launched with a sizable number of states and governmental entities, collectively covering about 80 million lives, using the initial NQF list of 27 serious reportable events. In the future, there likely will be even greater emphasis on the establishment of reporting systems to enable national learning and patient safety improvement. This report updates the 27 events identified in 2002 with current evidence and implementation guidance. One additional event has also been added. In addition, the report also summarizes the progress that has been made in implementing the list and provides guidance to those who will be engaged in implementing such reporting systems in the future.
